Prevent T2 – National Diabetes Prevention Program – Online
If you have been diagnosed with prediabetes or are concerned that you might be at risk for developing it (defined as having an A1C of 5.7 – 6.4 percent, or a fasting blood glucose of 100 – 125 mg/dl) consider joining this year-long program that focuses on diet and exercise to HALT prediabetes.
With the support of a lifestyle coach and your physician, lifestyle changes are made to reduce the risk of developing diabetes. This evidence based diabetes prevention program is supported by the Centers for Disease Control and is free for adults. This program has a $650.00 value for FREE compared to Weight Watchers.
- The 16 core sessions are held weekly for an hour and includes education online. Receive weekly email links to modules with tips and classwork. Your coach will reach out weekly with encouraging messages, quizzes, games and pledges to create for self motivation.
- The post core session meets monthly for 1 hour for 6 months online and your coach will reach out monthly.
- Class can be accessed on smartphones (iPhone/Android), tablets or computers.
