Prevent T2 – National Diabetes Prevention Program

Are you at risk for prediabetes?

Take this short risk assessment test –

If you scored a 5 or more, you may have prediabetes.

Prediabetes is a condition where blood sugar levels are higher than normal but not quite high enough to be diagnosed as type 2 diabetes. A person with prediabetes has an elevated risk for acquiring type 2 diabetes, heart disease, and stroke.

The good news is you can prevent disease progression to type 2 diabetes.  The Prevent T2 lifestyle change program was developed by the CDC and has been proven to prevent or delay type 2 diabetes by helping you lose weight, become more physically active, and reduce stress.

This FREE year-long Prevent T2 Program will start on Saturday, April 24 @ 10:00 am.  If interested in participating, please contact Deb Lehr, RN, Meritus Community Health Education & Outreach Coordinator at or call 301-790-8626.
