Living Well with Diabetes Self-Guided Home Course
Are you having difficulty keeping your blood sugars under control even if you have been living for years with diabetes? Are you searching for a way to refresh or learn more about how to best self-manage your diabetes, all in the comfort of your own home?
If so, consider learning more with the help of the Diabetes Tool Kit. With the regular guidance of a trained facilitator, you can learn about various tools that can help you manage your blood sugar while you self-pace your independent learning in a place of your convenience.
Once you have registered, an instructional tool kit will be mailed to your home and your facilitator will call to explain and to guide you as needed. Working together, we can help. Register by the 15th of each month.
